Peer Support Specialist is responsible for providing peer to peer support services including: monitoring, informing, supporting, assisting and empowering clients and their family members/caregivers who directly or indirectly receive behavioral health services; developing and coordinating activities, programs and resources which directly support clients and family members/caregivers in achieving wellness and recovery oriented goals; facilitating peer to peer assistance as a part of a team setting; conducting outreach to clients, family members/caregivers and the community; and acting in a liaison role between clients, family members/caregivers and community service providers.

Position Summary 

The Case Manager is responsible for providing direct services to clients referred to the CalAIM Enhanced Case Management Program. This position plays a critical role in coordinating care among multiple providers and addressing clients’ medical, behavioral, and social service needs. The Case Manager creates individualized care plans that meet health plan requirements and address barriers to care, including providing health education and coaching to promote long-term self-sufficiency.

The Case Manager also serves as the primary point of contact for a client’s care team, which may include primary care providers, behavioral health professionals, housing support services, SUD providers, and natural supports. If a client is dually enrolled in CalAIM Community Supports, the Case Manager may also provide Housing Navigation or Housing Tenancy services as needed.

• Develops and maintains relationships with Property Owners

• Promotes program through outreach calls, email, and presentations

• Liaises with referring agencies to streamline processes

• Interprets and communicates operating policies

• Coordinates collection and preparation of financial and operations reports

• Participates in interviewing job applicants and conducts orientation of new employees as needed

• Provides supervision and coaching to LLE team 

• Locates and compiles information; formats reports, graphs, tables, records and other sources of information

• Assists with special events planning

• Operates personal computer to access e-mail, electronic calendars, and other basic office support software

• Exercises administrative judgment and assumes responsibility for decisions, consequences, and results impacting staff, costs, and/or quality of service within the functional area

Minimum Qualifications (Knowledge, Skills, and Abilities)

• Bachelor’s Degree in Human Services or Business fields

• Minimum 3 years’ experience with program management and compliance

• Understanding of the Fresno County Department of Social Services and Fresno County Department of Behavioral Health is desirable

• Excellent communication skills (written and oral)

• Ability to work well in a team environment

• Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality

• Must be culturally/linguistically sensitive to populations served;

• Valid driver’s license 

• CPR Certification, First Aid Certification

ensuring the safety of residents and the security and sanitation of the program.

● Ensure a safe, secure, and efficient facility and program 24 hours a day.

● Ensures safety of residents and staff in all emergencies.

● Ensures residents’ compliance with program policies and rules.

● Engage with clients to create a welcoming environment.

● Pass out client meals and supplies.

● Prepare coffee for clients.

● Monitor grounds on an hourly basis for safety concerns.

● Conduct bag checks for all clients entering the facility in a safe manner and in

accordance with program bag check policy.

● Check for contraband in facility through general observation, room or package/purse

searches

● Utilize crisis intervention techniques to assist with de-escalation of situations while

prioritizing staff and client safety.

● Coordinate with Security Contractor for services on an as needed basis.

● Complete support documentation and daily shift reports

● Maintain resident sign-in/out log as well as all other logbooks and records.

homelessness.
